Qualifications for Purchasing Materials
The Neuropsychology Center sells Halstead-Reitan Battery tests and materials only to qualified professional users as stated in the Customer Qualification Policy. A completed Customer Qualification Registration must be on file before making a purchase.Placing an order
If this is your first order, you will need to submit a completed Customer Qualification Form. Only approved customers will have orders processed.Please use the online ordering cart to place your order. Otherwise, please contact us by email, phone, fax, or mail, if you wish to order materials.
U.S. orders: Visa, MasterCard, and Discover are accepted.
Alaska and Hawaii orders: Online payment is not available. Please submit your order and we will review it. Once we receive your order, we will calculate shipping and handling costs. We will then send you an email with order confirmation and payment instructions for your final approval. After receiving your approval and payment, your order will be promptly shipped.
International orders: Online payment is not available. Please submit your order and we will review it. Once we receive your order, we will calculate total cost including shipping and handling and any applicable taxes. We will then send you an email with order confirmation and payment instructions for your final approval. After receiving your approval and payment, your order will be promptly shipped.

Purchase Orders from U.S. government agencies or academic institutions may be accepted. Shipping/Handling charges (see below) must be included. Pre-approval is necessary for the use of a Purchase Order. Please contact us for pre-approval.
Prices
Prices are subject to change without notice. All prices are in U.S. dollars. All invoiced/quoted prices are good for 30 days.Shipping and Handling
Shipping and handling charges will be added to the listed price of the items at the following rates:Regular business shipping rates:
U.S. (contiguous): 10% of the item’s cost, minimum $15.
Alaska and Hawaii: 10% of the item’s cost plus specific shipping charges.
Canada, and Mexico: 10% of the item’s cost plus specific shipping charges.
Others: 15% of the item’s cost plus specific shipping charges.
Residential delivery: Additional shipping charges will be applied according to shipping method.
Regular shipping (U.S.) includes:
U.S. Postal Service or Fed Ex ground service.
Expedited or express delivery:
May be available for additional shipping charges. Please indicate on your order if you wish shipping to be other than our regular shipping method.
International (non-U.S.):
Please contact us for country specific charges. Buyer is responsible for any import/export fees, taxes, or other costs related to shipping and delivery of purchased items.
Shipment/Delivery Exceptions:
All shipments will only be made to valid delivery addresses as determined by the shipment company. Buyer is responsible for all costs due to incomplete or inaccurate address information, in-transit shipment modifications, and/or return of shipment.
Payment
All orders must be prepaid by check or money order in U.S. funds (made to “The Neuropsychology Center”) or by credit card (Visa, MasterCard or Discover). Wire or ACH transfers will incur a charge of $15 (domestic) or $25 (international).Returns
Returns of unused and unopened items are allowed up to 30 days following purchase. You must obtain a Return Merchandise Authorization (RMA) number within 30 days following purchase. The Neuropsychology Center will not accept returns without prior authorization and an RMA number. Once issued, RMA numbers are valid for 15 days. The Neuropsychology Center must receive the returned products within this timeframe. RMA numbers will not be extended or reissued. Customer should include the RMA number on a letter indicating reason for return. A full refund (minus shipping/handling charges and 15% re-stocking fee) will be issued once RMA items are inspected for condition.The Neuropsychology Center is not responsible for materials lost or damaged in transit. Further, The Neuropsychology Center is not responsible for unclaimed items, problems in transit, or other shipping or delivery issues. Purchasers must make a claim against the shipping company for materials that are lost or damaged in transit.
